The new patient health history intake form is required. You may fill out the intake form online before your first appointment, or in our office. This lets us know the past and current state of your health.

What to Expect at Full Circle

Dr. Choong will review your health history form prior to meeting, if they have been completed.

You will have a consultation with Dr. Choong to discuss your health related concerns. This is designed for Dr. Choong to learn more about you, your condition(s), and your expectations.

After the consultation, a chiropractic examination is performed. Neurological, orthopedic, postural, and physical tests will be performed. If necessary, x-rays will be taken.

On your second visit, a detailed report of all findings will be given and treatment options will be discussed.

If you are a candidate for chiropractic care, you may choose to experience your first adjustment at this time. Treatment is interactive. You may express concerns or preferences regarding the different treatment styles at any time.

*If you are transferring from another NUCCA clinic, you may not need new x-rays provided a copy of your listings have been received.
