What to Expect

On your first visit you will be required to fill out our new patient health history form if you have not already done so.

Next, you will have a consultation with Dr. Choong to discuss your health related concerns. This is designed for Dr. Choong to learn more about you, your condition(s), and your expectations.

After the consultation, a chiropractic examination is performed. Neurological, orthopedic, postural, and physical tests will be performed. If necessary, x-rays will be taken.

On your second visit, a detailed report of all findings will be given and treatment options will be discussed.

If you are a candidate for chiropractic care, you may choose to experience your first adjustment at this time. Treatment is interactive. You may express concerns or preferences regarding the different treatment styles at any time.

New Patient Health History Form

The new patient health history intake form is required. You may fill out the intake form in our office, or you may request a copy of the form be emailed to you in order to complete it before your first appointment. This lets us know the past and current state of your health.
